Firstly you need to get your two images!. Obviously the first one will be with the hand in, but the second can either be done through various filters in Photoshop or simply a scanned image of writing you have done yourself on a piece of paper (try and make sure the colour is black cos its slightly easier)

Then to open your picture (main pic with hand) and create a new layer!. Open the image with the writing and using the move tool (V) drag and drop the image onto the main pic!.

Then scale and move the image to roughly where you want it positioned (if it has a white background don't worry)

go to edit > transform to give you some options to either skew or perspective etc!.!.!. this should help position it in place realistically!.

Go to layer blending options (on the layers palette) and click on 'overlay'!. Try a few others to see if they do the job better!. By now the white background should have gone and you will be left with the tones of the hand making an effect on the text layer!. You can use opacity to make it more realistic!.

To add a greater realism go to filter and then liquify!. Use the hand smudge tool to slightly push and pull peices of the text layer over countours of the hand!.

Finally use the transform tools to slighly modify or layer> layer mask> reveal all to remove any pieces of the text layer that you wouldn't be able to see from that angle!.

Hope this is of some help!

Matt!.
