Contemporary: This is an all encompassing term that generally includes art work that has been done since the latter part of the 20th Century, after the end of the Modern Era!. But as the name implies, anything that is current can be considered contemporary!.

Modern: Generally considered to be the period from about 1905-6 to the mid-1950's, when Pop art ushered in what is referred to as the postmodern period in art!. Modern art is generally characterized by formal experimentation and exploration, and mostly seriousness of purpose!.

Christies define contemporary art as anything after 1970 and Sotherbys defines work created between 1945 and 1970 as 'early contemporary' and work created after 1971 as 'late contemporary'!.
Modernism was an artistic movement that took place during the late C19th and early C20th and was characterised by a move toward abstraction, the concept of the 'innocent eye' (that each work is looked at with a 'fresh' frame of mind, that the viewer leaves their preconceptions at the door) and a need for originality among many other things!.
Contemporary art is just art that is current!.
