Best Answer - Chosen by Asker:
First ask them what their normal clothing size is and record that!. and then do this:
Height!.
Chest/Bust
Waist
Hips
Shoulders - across the widest part of the back!.
Arm length from top of arm to wrist!. (You will adjust this according to length of sleeve, with about an inch or two longer for most long sleeves!.
Nap of neck to natural waist!.
For men and sometimes for women depending on the costume take measurement for neck!.
And then the length of the item you are making!.

For men, pants are very hard to do!.
you need what is called the rise!.!. front of pants from top of waistband to the point where all 4 seems (the back, front, and top of each leg) meet between the legs!. Just use a pair of pants that fit them and take the measurements from that!.
also inseam and the outside seam for men's pants!. The outseam is from the waist to the bottom of the pant leg, wherever you want the pant leg to end!. The inseam is from the crotch to the bottom of the pant leg!. Again this is easier to do if they give you a pair of pants that fit well are are the right length!.!. you can just measure that instead of the person!. (Male taylors have an easier time of getting these measurements than a woman would, of course!.)

How to do it:
Have them stand up straight!. Use a cloth tape measure and do each twice to be sure that it is right!. Do not round off the numbers, be as accurate as possible!.

If hats are being made!.!. take measurement around the widest part of the head!.Www@QuestionHome@Com
