It means that the narrator wants to know if you would remember him if you saw him in heaven!. Would the relationship or friendship etc would be the same as it is now!.Www@QuestionHome@Com

When he asks "Would you know my name" he is asking if his son will recognize him, because a long time will pass before Eric expects to die!.
