You might try contacting your tribe of enrollment (assuming you are enrolled)!. They'd be able to assist you assuming they have a copy of your original!. Otherwise you'll probably have to go through your local Bureau of Indian Affairs field office to learn how to obtain a copy of your card and apply for one for your daughter!. See http://www!.doi!.gov/bia/leaders!.pdf to find your local BIA office!. Regarding using your CDIB for medical services, you'd have to go to an Indian Health Services clinic!. No doctors I know of have ever accepted my card as it is not an insurance card!. When you think IHS, think HMO!. You will have to go to an IHS clinic and get a referral to an outside provider, assuming that outside provider has agreed to accept IHS as a method of payment!. Good luck!Www@QuestionHome@Com
