The most common would be Janieski, but there's also Janaski or Janasek, Januski, Dzunieski (DZ makes a J sound), Drzewicki, Janawski, Janezewski, Janusiewski, Janecki, Janiczki, Janiewski. Of course, each region has its own dialect, so there are a few extra consonants and vowels that could slide in here and there. But this is a pretty decent place to start.
