Question:This is a copy of my grandmother's death certificate. I have been unable to decipher the handwriting for #13 (father) and #15 (mother's maiden name). Can anyone help? Thanks!

This form was filled out twice with inaccuracies.

Consider the typed entries firast: Pheannie Lewis age 82, died Dec 23, 1930 but attended by the doctor Oct 1 to Dec 23, 1931; died of Pellagra complicated by starvation.

Now look at the the inked answers:

Married to Hughie Lewis; birth Oct 18-something
Age: 1948, Oct; Housewife born in Pike county, Miss (Mississippi).

Father's name is: Thenie Samson
Mother's Maiden name: Thenie Samson
born Pike County, Miss (Mississippi)
Informant: Taylor Ark

I would assume that the typed entries came in first -- and even those have the dates wrong.

If Pheannie was 82 in 1931, then she was born in 1849 (which fits with the miswritten 1949). If she had a child who help fill in the form (inked part), they may have misunderstood that they were to give Pheannie's parents' information and gave Pheannie's instead -- born Pike Co, MS.

With no actual place of death given (house, hospital) having the informant listed as "Taylor Ark" would indicate to me that the information was from hospital records or notes taken by some city agency.

I can read very little of it..you should ask someone in person..I don't think it's a good idea to be posting this..
Have you tried pressing the control and plus to make it bigger and then control minus to make it normal size again.
Using this method I can make out Miss, and I thought at first the name was Thelma, but using this method I can see it isn't. I shall examine it again and see if I can make it out for you.
Thenis....What nationality are you from??? because, no one's maiden name should be Thenis.
Looks to me like #15 is Thenis Sawson or Cawson

#13 Phennie Cawson or Sawson b. 10/2/1880 in Pike County, Mississippi
Strange that the same name is given for father and mother.

Father-Thenie Saurson
Mother-Thenie Caurson

I also noticed that the first name of the deceased is Phennie which is very close to Thenie do not rule out the same spelling of the first name for this person.

I also noticed that the death date of Dec 23, 1930 may be incorrect. The doctor claims to have attended this person from Oct 1, 1930 until Dec 23, 1931.
