Best Answer - Chosen by Asker: The office of vital statistics in your state's capitol city. You will have to provide identification , probably at least 2-3 different things such as driver's license, social security card etc. There is a small fee.

I made one phone call to Baton Rouge, Louisiana bureau of vital statistics, they asked me to fax those things and then I gave them a credit card number so they could fed-ex my birth certificate. I had it in 4-5 days.
