Question:

How do I find a death certificate with limited information?

My great-grandpa died in 1960. I don't know the month or year. It was somewhere in NYC. Unfortunately, this is right before they started standardizing SSN death records. How do I find his death certificate with such limited information. I know his full name obviously. The reason no one knows the other information is because he left the family and died alone.

Best Answer - Chosen by Asker: Contact the New York City Department of Health and Mental Hygeine. They have Death Records since 1949. The death records are restricted to family members and the cost is $15. The staff can find the Death Record as long as you know he died in 1960 in New York City.

You will not be able to search for the record yourself since New York restricts records for deaths from 1949 to the present.

See the second link below for an application you can mail in or bring in person.

You really shouldn't have any difficulty obtaining this record with the information you have as long as the information is correct.
