I have a document from 1915 in CZECH. that I need interpreted.See below.?

This document is from my mother whom was born in Czech. It is headlined KR(WITH a ^ mark ABOVE THE R)ESTNI' LIST.
I might be able to use my scanner to send this to the person or persons that may be of assistance. A couple of dates are also shown as 1912. Also it is CERTIFIED and stamped with a round certificate with the following letters: SIGNUM PAROCHI/E EVANG.A.C. SISNEN and possibly ESV. There is a CROSS in the middle of the stamped certificate. THE CROSS IS AN OPEN ONE AND MIGHT BE OF A CROATION CROSS that I have seen somewhere in my life. Please email me if you can lend assistance in this matter. Regards, mrpagelray@yahoo.com

Their site says: "This free translation service is dedicated to texts related to genealogy". Their site also says that Volunteers do translations. Czech is listed as a language they translate.
