Just trust in yourself. I went through a similar situation myself (16 years of ballet, tap, and jazz; with 3 years off). It was a really difficult time, because I was terrified of looking badly. Just trust yourself, and ignore those feelings of nervousness.

To get better at ballet: Focus on the basics. Get them back into your muscle memory and comfortable doing them again. You'll find that the more difficult and complicated steps will start coming back and feeling natural again.

For hip-hop: Again, the basics are key. Focus on making your movements as exact as possible. The sharper you are, the better your dancing will be. The ballet lessons will give you a great foundation.

Most of all, dance with your heart. No matter how nervous or uncomfortable you are, always put all of your passion into your dancing. Even if you make a mistake, you'll look and thus feel so much better than if you were just marking the steps. Soon, you'll find all of your previous experience coming back.

Good luck!

try taking a class with a group so you can all dance together, that way you can learn tips from people like yourself and not have to worry about constantly being watched because you are the only one dancing.
Jazz dance is very fun and kind of like ballet, but of a more relaxed nature.

If your passion is in hip hop, do that. If you practice enough, you'll improve. Try watching some youtube videos for insperation.
