Writers block- What should happen next in my story?

I am to a part where this guy, Dylan, has a brother, Caleb, who is dying. He decides not to sit in a hospital E.R. waiting for his death, so he goes to his girlfriend's house.
The story is like a drama. It has had car crashes and kidnappings, so I want something dramatic, but please, DO NOT SAY RAPE
