Ala Wikipedia & Wiktionary

Perception: The process of acquiring, interpreting, selecting, and organizing sensory information (in this case it would be visual) and conscious understanding of something.

Conception: An image, idea, or notion formed in the mind; a concept, plan or design.

This means that the conception is what the artist intends to make, and the perception is how someone sees it.

It is worth noting that an artists conception of a work might not end up being peoples perception of it. There is actually a large debate whether the artists conception or the general publics perception determines the meaning of a work of art.
