The person in the background is Lot's wife turned to a pillar of salt, she has turned back to see the city of Sodom in flames!. As is quite common in religious and mythological paintings , several scenes in a story are shown in one painting that is the story of Lot's daughters and Lot's wife turned to a pillar of salt!. !. Or!.!. it could be (and I could be completely wrong here), as we remember the story of Lot and his wife, a reference to her!. She looked back when God was destroying Soddom (as the angels had advised against)!. So she was turned to a pillar of salt!. Www@QuestionHome@Com

I rather think it's the image of "guilt",!.!.!.!.bald and thin and alone!. I could be wrong,!.!.!.it could actually be a demon trying to influence Lot by blinding him with wine!.Www@QuestionHome@Com
