Medium refers to the material it was made from. Paintings can be oil, acrylic, watercolor, tempera, or a fresco in/on plaster.

Sculpture mediums could be cast bronze, marble, ceramics, wood, glass, an 'assemblage' (lots of things collaged together) or anything really!

The medium is usually referred to material artist used to create the work.

For example when they say the medium for the painting is oil on canvas, that means the artist used oil based paints to paint on a canvas material/surface.

Its the materials that are used for a painting, for instance oil paint on canvas, or frescoe.

Mixed media refers to collage and that type of art where several different types of materials are used to comprise the work.
