To smooth skin:
Be sure you have your history state palette window open.
Open your photo.
Go to Filter - Blur - Gaussin blur
Apply enough blur to greatly blur the photo but still be recognizable.
Then in your history window, go back up to your open state. The photo will no longer be blurred.
In the state under the open state, (the gaussian blur state), place a check mark in the little box beside the gaussian blur state.
Then get the history brush from the tool bar. Use a soft brush. Set opacity to about 15%. Then brush on the skin. It is an additive brush, the more you brush the more "smooth" the skin will become, (you are actually adding the gaussian blur in tiny, controllable amounts). Of course, do not brush on eyes. Just apply enough softening only in areas where needed. You do not want the overdone, phoney plastic skin look. Knowing when to STOP is just as important as knowing what to do.

For the border:
Go to Image - Canvas size
Be sure RELATIVE is checked
You can then add the border in either pixels or inches. Look in the color list and you can choose white, black, or background, or click on the little square box to select any color you want.
When you click OK, the border will be put around your photo. You will probably have to undo it a time or two until you get the size exactly as you want.

CS3, while I am using CS2, nevertheless, I am sure it is similar

To make the face appear smother, check out the filters. Things like Gausen (sp) blur can help.

Also, go into saturation, sometimes removing some red will have that affect.

As far as putting a border around a photo, chose Image, then canvas, then make the size bigger and then chose a color.

There are a ton of photoshop trick websites, you may want to do an internet search to find some good tricks.
