As a 77 year old, married some 55 years I often come out with similar thoughts "How do I tell my wife I have forgotten...". But I usually manage to say what I have to say.
In my dotage I can be excused (at times) for being a bit over critical, though I hope not in your case.

First, your poem has potential, potential because I do believe it is not finished. It cry's out for at least three more verses. You seem to be holding back all you really wish to say.

You have not opened your heart enough, for your feelings go deeper than what you have expressed.
This poem is from a woman whose feelings go no deeper than a fleeting relationship. Be the woman you are and tell him ALL that you feel. Let your passions flow freely.

Change the tempo of the traditional set poem and break it up with something like this.

How can I tell this guy
I have waited for him
Have cried for him
Have longed for him.
Or shall I leave it all up to him?
Only kidding with the last line, but do you see what i am getting at?

May I also suggest that you change the title to "This guy' the same as in the body of the poem.

I agree. Show him this poem. If you dont want to be blunt about it. Bring up boyfriends randomly in a conversation. Stare at him accationally. Then tell him you wrote this poem and you wanted to see what he thought of it.

i agree you should show him this poem or read it to him one or the other
