From birth till death your body (which is comprised of a multitude of cells) diligently maintains your homeostasis by replacing dead and dying cells with new ones!.
This process depends upon the molecular structure of DNA!.
What happens is that free radicals damage this molecular structure and the replacement cells become less effective over time!.
When the DNA divides it makes a copy of itself!.
If it has been damaged then the copy will be damaged also until after a certain point the copies are no longer able to maintain homeostasis!.
Of course this is not the only cause of death but it is an inevitable one at this point!.
Others would include sever trauma to certain organs in the form of decease or direct physical destruction!.
I/me or the "you" in your questions format, am dependent upon the process that maintains the entity is refered to as self!.
The principle manifestation of this concept is driven by the my brain and its functions!.
If the body dies the brain dies!.
If the brain dies the self will perish also!.
The concept of a soul or self that can exist independently of this process of a physical body questionable and thus I do not accept it based upon arguments I have encountered!.

Scientific: Because bodies break down!. We can keep replacing bits, but either medicine or your bank account will run out of ways to sustain your existence!.
