First of all, don't try to stop your voice from going into another register. Let it crack for the moment. When you practice scales, let the voice go up - don't manipulate it.

Make sure you are taking a low breath before you sing - don't forget to breathe.

Your jaw, tongue and cheebones should be relaxed, not tight, when you're singing. When you're singing, massage your jaw and cheekbones to keep them loose.

Think of your high voice going out through the top of your head into a higher plane - assume a "yawn" position where the soft palate is up, the tip of the tongue should be against the teeth, not paralyzed somewhere in the middle of your mouth.

To get the high note placement, when you're first singing or practicing, lean over. This will force the palate up. When you go for a high note, pretend your lifting a piano.
