For Artists:
They are called "Artist Bio Kits"

For Models:
They are called "Modeling Portfolios"

An basic Artist Bio Kit includes:

1. 1 B/W ( black and white) 8" x 10" headshot photo of you
2. Brief 2 or 3 page biography of your music background, etc.
Make sure you put your Myspace links ihn your bio too so the Record Labels can look for themselves on you.
3. Your Cd demos and DVDS if you have them.
4. Any newspaper clippings/ reviews or awards you have received in your music career.

Artist Bio kits and Modeling Portfolios are not the same thing at all. They are completely 2 separate things entirely.

A basic Modeling Portfolio insludes:

1. A big Avery 3 ring Binder with many clear sheet protectors inside to keep your modeling photos free from dirt, etc. Like a huge Photo "scrapbook" sort of. They are full of all your current Modeling photos from your Modeling Career.

I know this because I used hang out with a Real Modeling Photographer years ago. He showed me all his models pics in his Modeling Portfolios. I'll say that he had some very beautiful women in those modeling portfolios for sure.
He was very, very picky about who he took photos of.
He charged each model about $110 to make a Modeling Portfolio for them. I think that was his going rate at the time, not %100 sure though. Every Modeling Photographer charges different rates so you really have to do your homework and research and find out their standard Portfolio price rates. Good luck

A portfolio is basically the artistic version of a resume.
Include pictures of you and any musical acheivements.
Ie "Played Sandy in Grease production at ___ theatre" etc.
Your demo tapes would be part of the portfolio.

It may help to find, make an appointment with, and try to get in with a talent agency. They should help you put together a portfolio. If you plan on singing you might want to start off with head shots and stuff. You will have a sheet with your name on the top and your experience and etc etc. I'm getting mine made soon.
Find someone that may help you put one together.
Just surf the net about it and look in your phone book and stuff. I'm sure there's something. I hope this helped a little.

With technology, Just record a CD with your songs on the tracks. Then, whoever is auditioning you can go to the song they want with ease. Couple that with a good resume' and you should be set.
Some of my more advanced singers make a Pop cd, a country cd, a classical cd and a broadway cd. Then they can bring the right one for the venue they are auditioning.

Learn lots of tunes, hire an accompanist to do a recording session with you, one tune after the other and then burn cd's later.
