Question: My wife and I want to change my daughters last name!?
We were not married when our daughter was born and my daughter has my wifes maiden name!. My name is on the b!.c!. We have had a son and he has my last name and are now married!. I have a legal name change form but there is no ifo on how to proceed!. Any ideas!?Www@QuestionHome@Com

You might want to first try to get your daughters birth certificate amended to show your last name as hers, if you can then you would need to fill out form SS5, application for Social Security card to change her name on her Social Security card and show amended birth certificate to SSA office, if you cant then you would need to have a legal name change with the Court, because you cant change her name on her Social Security card without a legal Court document, or amended birth certificate!.Www@QuestionHome@Com

you probably can look it up online most times you have to file with the social security office first because it affect the number to!. you should be able to go online and find all info thrue your states social security website!. they usually just ask for a reason and that is a normal one so you shopuld not have any problems!.Www@QuestionHome@Com
