Short answer - usually the mother's surname!.

Longer answer - What country are you thinking of!? The answer may vary because of that!. But in general, kids are put into the care of the state for many reasons!. Few kids are simply abandoned as infants with no clue as to who their parents are!. Perhaps the parents died or the state had to take the children for abuse or neglect!. The idea that most kids don't get adopted (or end up in some kind of more permanent situation like a guardianships) will also vary by country and the child's situation!.
