How do I add a name to my own name?

Question:Okay so example say I'm called John Smith right, and I wanted to add a middle name like say 'Jason' so I would be called John Jason Smith where would I go about doing that?

any attorney can arrange that for you if you want the name to be legal

Just add it, who would know it wasn't your name. Or you can have your name changed legally, maybe through a lawyer.
