Question:If you believe it actually is a name, it may be a shortened name. Yamaguchi might be shortened amongst friends so the person becomes yama-chan or simply yama. Without seeing the kanji its difficult to tell, but it def doesn't look like a complete name be it a 1st or last. (my guess would be a shortened last name).

Never heard of kosa as a first or last name in Japanese.

Actually, to my knowledge people are named by the literal meaning. If Kosa is a clan, that it's a last name (first/family name) I've never heard of that name though.
