my father's grandfather had changed his last name before my time.how could i legally change it back to the original?

Best Answer - Chosen by Asker: A district court and in New MExico the cost is 135 dollars I think that is pretty much the standard fee. My partner (we are gay) and I wanted to change our last name os that it included both names, we used forms we got from the following website http://www.namechangelaw.com/ They have forms for every state You can do it yourself or they will do it for you, we did it ourself with their forms and it was a breeze.
