Where to locate Baptismal records?

I'M TRYING TO FIND OUT WHERE TO ORDER A COPY OF MY BAPTISMAL CERTIFICATE. I KNOW IT WAS DONE IN HOLLYWOOD, FL. IN 1979, BUT I DON'T KNOW THE NAME OF THE CHURCH OR ANY OTHER DETAILS.

Best Answer - Chosen by Asker: Well, unfortunately, I think the only place that would have any forms like that would be the church itself. The government wont have them, I can guarantee that.
