My birth certificate names are mess up. how do i fix this?

on my birth certificate my last name is RUSSELL which is my mother last name, my dad last name was MILLER . i have been using MILLER for my last name since i was in grade school. now i'm 50 year old and i need a passport, all my id's are in MILLER, but my birth certificate is RUSSELL. what do i do?

Best Answer - Chosen by Asker: You need an affidavit stating the facts and that should do it. If your mother is alive she can do that and I think that will be even better.
